    An Asian police officer who raised concerns about a "racist" toy monkey at work was accused of gross misconduct, BBC News has learned. The officer said he saw the black toy wearing an officer's shirt at Belgravia police station in central London. Police chiefs promised to look into the matter but the detective was himself investigated for making the story up. He was later cleared at a misconduct hearing and received damages from the force after taking legal action.     A driver who started an argument with two cyclists only to discover they were off-duty police officers has been fined £200. Joseph McCarthy, 50, had pulled in front of the cyclists before braking unnecessarily in Larbert's Bellsdyke Road, causing them to brake. McCarthy was later identified, reported and charged by the officers. He was convicted of careless driving at Falkirk Justice of the Peace Court and had three points added to his licence.     More details now on the crash on the A41 at Berkhamsted - it's been confirmed that two people have died and a boy has been arrested. The crash between a silver Nissan Micra and a silver Peugeot van happened on the northbound carriageway between Tring and Chesham at about 23:25 yesterday. Two people who were travelling in the Micra died at the scene. Two other people sustained minor injuries. A 17-year-old boy has been arrested on suspicion of two counts of causing death by dangerous driving and is in police custody.     A police officer has been charged with theft, forgery and misconduct in public office. Merseyside Police Sgt Colin Hughes, 34, is accused of forging a document, making false statements and stealing a machete that had been seized. He is alleged to have carried out the offences as a temporary inspector between July 2014 and December 2015.     A 12-year-old boy has been charged after a police officer was racially abused and then spat at in the face. The boy was arrested following the incident on Drake Street in Bradford on 3 May. He has been charged with a racially-aggravated public order offence and with assaulting a police officer.
